Instrument Details🔗

The Medical History Questionnaire is a questionnaire developed by the physical health working group members that assesses medical diagnoses, exposure to medications surgical and therapeutic interventions, vaccine behavior, dental health, insurance coverage and injury prevention behaviors. Of note, injury prevention questions change at each visit to correlate with preventable injuries at specific ages.
